Consent

We obtain explicit consent to keep your personal data and allow us to provide travel clinic services. Our clinician will explain both points, individually, when you visit the clinic before commencing your treatment plan.

Privacy Notice

We process your personal data, which includes your name, contact details, medical history, prescription medicines and data from healthcare services we provide to you such as travel risk assessments, vaccinations and travel medicines for the purposes of:

Your care– providing pharmacy-based travel vaccinations, general health vaccinations and travel medicines.

Management – We only keep relevant information as detailed above. We do not share information with any other bodies unless permission is granted by the patient or legal guardian or we are legally obliged to do so and this would be limited to GPs, other NHS bodies and Local Authorities if appropriate.

We hold information for as long as required using guidance from the NHS and Health and Social Care Information Centre, now known as NHS digital.

For children and young people retained until the patients 25th birthday or 26th birthday if the young person was 17 at the conclusion of the treatment all other records are kept for 10 years as determined by www.igt.hscic.gov.uk. You have the right to a copy of the information we hold about you, generally without charge. You may seek to correct any inaccurate information.

We process your personal data in the performance of a task taken in the public interest, for the provision of healthcare and treatment and the management of healthcare systems. A pharmacist is responsible for the confidentiality of your information. You may object to us holding your information. You may also lodge a complaint with the Information Commissioner’s Office, Wycliffe House, Water Lane, Wilmslow, Cheshire, SK9 5AF. Please ask for more information.
